For this gasket as well as the 02 gasket, is rtv silicone needed?

  No, absolutely not. Anything you put on this manifold gasket would smoke like crazy and most likely catch fire. This is to be put on just the way it is, naked! Its a steel gasket.

Which side of the gasket faces the head?
There is a flat surface and a raised surface. Which side does the flat surface get applied to?

  The metal side goes up against the cylinder head, and the carbon lined side goes towards the turbo manifold.
